Awards & Medals
Jackson Medal - To apply for the
Jackson Medal, complete this form and send it to one of the Jackson
Medal Committee members listed on the form.

War Service Medal and the Southern Cross of
Military Service Medals - Note that this form cannot
be completed
on-line. It must be printed and completed by hand.

The War Service Medal (on the right in the image) is for those
veterans who were in combat. The Southern Cross of Military
Service (on the left in the image) is for veterans who were
never engaged in combat.
Each recipient will get both a
full size and a miniature version of their medal.
The same form is used for both medal applications. An
application form
must be
submitted to and approved by the
Awards Committee
in order to
purchase either medal.
Chapter Level Forms
Change of Command - (Also
used at the Society level) - Form for reporting the changes in Chapter
or Society Commanders, Lt. Commander or Adjutants.
Annual Chapter Report
- Used by local
Chapter commanders or adjutants for the annual report of the Chapter.
Chapter Recommendation For Recognition
- Form used to recommend Chapter members for recognition and awards to
the Society Commander.
Last Commission
- Form to report deaths in the
Chapter.

Society Charter

Annual Society Report - Official
Report: The report of the Society Commanders.
Annual Army Report
- Official Report: The report of the Army Commanders.
Nominations For Awards - Report of
the Society Commander to the National Awards Committee.
Chapter Evaluation - Form used by
Society and Army Commanders to evaluate the relative health and
functioning of a Chapter.
